CHAPTER 3: Axial and peripheral clinical manifestations of axial spondyloarthritis

Abstract

Spondylarthritis (SpA) can present with predominantly axial or peripheral involvement and is termed axial SpA (axSpA) or peripheral SpA (pSpA), respectively. AxSpA primarily affects the sacroiliac joints and spine, with ankylosing spondylitis (AS) or radiographic axSpA (r-axSpA) being the prototype of these diseases. Prevalence is difficult to establish due to the ethnic heterogeneity of the population, the lack of feasibility of applying current criteria (HLA-B27 testing and imaging studies such as pelvic radiographs and magnetic resonance imaging), and the transient nature of some SpA symptoms (peripheral arthritis and enthesitis). According to the National Health and Nutrition Examination Survey (2009-2010), the prevalence of axSpA is estimated to range from 0.9% to 1.4% in the adult population in the United States. In Latin America, through a review of the literature, the prevalence of SpA and its subtypes was determined, which varied from 0.28 to 0.9% (SpA), 0.02 to 0.8% (AS), 0.2 to 0.9% (axSpA) and 0.004 to 0.08% (psoriatic arthritis - PsA).
