CAPÍTULO 13: Rol de la ultrasonografía para el estudio de las espondiloartritis 2: articulaciones sacroilíacas
Resumen
Las espondiloartritis (EspA) representan un conjunto de patologías que comparten ciertas características clínicas y genéticas, cuyo prototipo de enfermedad es la espondilitis anquilosante (EA). Las articulaciones sacroilíacas (SI) presentan características únicas desde un punto de vista anatómico y funcional, que dificulta frecuentemente la evaluación del compromiso inflamatorio (sacroilitis) mediante el examen clínico y los cambios radiográficos suelen ser tardíos, llevando a retrasos en el diagnóstico. Las radiografías convencionales de las articulaciones SI tradicionalmente se han usado para el diagnóstico, clasificación y monitoreo de las EspA axiales, siendo la sacroilitis el componente central de los criterios diagnósticos para EA. Las radiografías de las articulaciones SI no sirven para el diagnóstico temprano, y debido a la alta variabilidad intra e interobservador, pueden provocar resultados falsos positivos y negativos.Citas
